DELIVERY SLIP WITH GUARANTEE LETTER

PROBLEM TO BE SOLVED: To provide a method for issuing a form and a guarantee letter, which enables attachment of the guarantee letter to an article of commerce dealt by mail order at its delivery without unpacking. SOLUTION: This delivery slip consists of a first sheet and a second sheet pasted to the first sheet. The second sheet is a sheet consisting of at op paper and a releasing paper provided through a self-adhesive layer on the under side of the top paper and equipped with a pasting slip and a delivery slip. The guarantee letter is connected to the delivery slip with a releasing layer provided in a region between both the sheets corresponding to the delivery slip.
