PROBLEM TO BE SOLVED: To connect an optical fiber with an optical waveguide and excellently align an optical axis by an easy method without increasing the number of parts. SOLUTION: The device is provided with at least a base plate 1 having a prescribed refractive index, an optical waveguide part 2 composed of a material having a refractive index which is larger than that of the base plate and replacing a part of the base plate and beam transforming parts 3 and 4 which are located at both the incident part and the emitting part of the optical waveguide part, made by replacing a part of the base plate with a material having a refractive index which is larger than that of the base plate and have a finite radius of curvature at the border of the base plate.
