PROBLEM TO BE SOLVED: To realize a high precision surgery or a raw material processing that is excellent in operability and operation efficiency by outputting a desired synthetic oscillation of a vertical torsion in an ultrasonic wave horn-tip by processing vertical oscillations transmitted from an ultrasonic wave oscillation mechanism. SOLUTION: The hand-piece is equipped with an ultrasonic wave oscillation mechanism that outputs ultrasonic wave oscillations, a horn that amplifies oscillations transmitted from the ultrasonic wave oscillation mechanism by being connected with the ultrasonic wave oscillation mechanism, and a ditch part to convert the vertical oscillations transmitted from the ultrasonic wave oscillation mechanism into synthetic oscillations of vertical torsion. The ditch part is formed more than one on the above-mentioned horn and/or on the peripheral face of a member which is connected to the horn, and in addition, the purpose of this invention is solved by the ultrasonic wave hand-piece which is characterized in that the ditch part is provided in the vicinity of the stem position of the torsional vibration.
