PROBLEM TO BE SOLVED: To provide a medical examination slip which shows objective ranking of an attainment level of target medical examination items, and a diagnosing method. SOLUTION: This medical examination slip has a diagnostic item column 2 showing a plurality of diagnostic items 3 per step, a judgement column 4 showing the selection of alternatives by the words 'yes' or 'no' to the diagnostic items column 2 and an evaluation level column 5 setting forth a plurality of rankings to the judgement column 4. The alternative 4y shown in the judgement column 4 is related to the diagnostic item 3 as a next step of the diagnostic item column 2, while the other alternative 4n is related to a specified ranking of the evaluation level 5r. In addition, both alternatives 4y and 4n of the judgement column 4 corresponding to the diagnostic item 3 as a final step of the diagnostic column 2 are related to specified rankings of different evaluation levels respectively. |
