PROBLEM TO BE SOLVED: To provide a two-wavelength multiplexer/demultiplexer module having a high isolation. SOLUTION: The two-wavelength multiplexer/demultiplexer module is provided with an optical waveguide substrate 1 having an end face 12 which is inclined by approximately 45 deg. with respect to the incident/emitting optical axis, a groove having V-shaped cross section 13, an optical fiber 131 fixed on the groove having V-shaped cross section 13, an optical filter 3 which is composed of a dielectric multi layer filter arranged on the extension of the incident/ emitting optical axis of the end face 12 of the optical waveguide substrate 1, either of a laser diode 5 arranged opposite to the optical filter 3 on the extension of the incident/emitting optical axis or a light receiving element 4, either of a laser diode 5 arranged opposite to the optical filter 3 in the direction perpendicular to the extension of the incident/emitting optical axis or the light receiving element 4, a linear optical waveguide 6 arranged between either of a laser diode 5 or the light receiving element 4 and the optical fiber 131 and a linear optical waveguide 6' arranged between either of a laser diode 5 or the light receiving element 4 and the optical filter 3.
