PROBLEM TO BE SOLVED: To provide a form apparatus capable of suppressing protrusion from a continuous footing and of accurately and easily building the footing at a low cost, suitable for a store or the like. SOLUTION: This form apparatus is equipped with a foam body (10), having a pair of side walls (13) and (13) for forming a rising portion of the footing and a partition member (15) for forming a region, having no concrete fill between the partition and one side wall (13) in the upper portion of a space inside the form body (10). The partition member (15) is located almost at the center inside the form body (10) in a state supported by a bearing device (20). By using this form apparatus, a continuous footing (60) with a corner cut off at the indoor side of a non-structural part of the footing rising portion can be formed easily.
