<p>PROBLEM TO BE SOLVED: To provide a means for making it difficult to forge and for verifying the contents of a contract, a contract document enabling acquisition of the information on a customer being a contractor and a method for concluding the contract. SOLUTION: In one form so functioning as having two roles when it is divided at a cut-off part, so as to attain the purpose, an optical diffraction structure is formed in each of the two forms divided at the cut-off part, and a code proper to the form before it is divided is printed visibly on each of the two optical diffraction structures. The method for concluding the contract wherein a recovering means for recovering a written application of the contract wherein the information proper to an applicant of the contract is entered and a means for registering the contract wherein the recovered information proper to the applicant of the contract and the beginning day of the contract are registered as the information on the contractor are executed, is provided by using the form.</p> |
