PROBLEM TO BE SOLVED: To provide an optical disk adjustment device which has excellent reliability and easiness for handling, in which adjustment of the emitted light characteristics, the intensity distribution characteristics and three beam angles or the like of an optical head are performed with a simple mechanical structure. SOLUTION: An optical head adjustment device 30 has a grating plate 32. The grating plate 32 is composed of: a base plate 38 and has a diffraction region 40 which forms an interference image by diffracting light which is made incident on the base plate; and a non-diffraction region 42 which transmits the light which is made incident on the base plate without diffraction. A moving mechanism 36 of the adjustment device 30 moves the grating plate relative to the transmitted light in the direction approximately perpendicular to the transmitted light of the optical head at least from a first position where the transmitted light is made incident on the diffraction region to a second position where the passing light is made incident on the non-refractive region, or from the first position to the second position.
