FIELD: medicine, neurosurgery. SUBSTANCE: one should resect the external leaf of dura mater (DM) 2-3 cm above and below craniovertebral transition (atlanto-occipital membrane), meningeolysis is carried out 0.8-1.2 cm for both sides against the incision to tighten up an internal wall of dural sac in dorsolateral directions with the help of handles by external lateral leaves. In area of craniovertebral transition DM is fixed from ventral side to an atlas body and cranial slope. Handles are sutured to periosteum of occipital bone scale. To prevent further lowering of cerebellar hemispheres into osseous defect it is necessary not to continue DM resection upwards above cerebellar hemispheres by limiting it with area of occipital-cervical transition only. The method enables to remove the compression of brain trunk, restore a large occipital cistern and improve cliniconeurological picture of the disease in patients with Chiari's anomaly of type I.
