A forming/film−forming apparatus (700) forms a disk substrate from a stamper (330) formed from an optical display blank (320) and forms a coating film, e.g., a reflective film on the disk substrate to fabricate an optical disk (340). An additional recording apparatus (200) additionally records additional information on the recorded optical disk (340) fabricated by the forming/film−forming apparatus (700). Additional information is recorded on a portion detectable as a pit when a laser beam is applied to a predetermined area of the recorded optical disk (340), and thus an optical disk (350) where the additional information (420) is recorded is fabricated. In such a way, identification information for identifying the optical disk (350) is additionally recorded.
