<p>PROBLEM TO BE SOLVED: To drastically shorten the time required for the whole formation of care plans in a formation support system for care plan for insured persons of nursing care insurance and to perform the formation and maintenance of a proper care according to the state of each insured person regardless of the experience or ability of a care manager forming the care plan. SOLUTION: On the basis of common item data and additional information for each insured person inputted by a user (S1 and S4), and an in-home service plan that is a document drawing the whole care plan of each insured person and a service using form and service providing form for each insured person are formed on the basis of selected problem to be solved, nursing aim, and contents of nursing service (S6, S7, and S9).</p> |
