When the statement slip is folded along the fold 2, it changes quickly into the FAX order sheet 1. Since the border, the column, etc., corresponding to the surface are printed beforehand in the FAX order sheet 1, items such as a customer's name, a code, a name of a commodity and a unit printed or hand-written in the surface column can be used for the order sheet. Therefore the time for writing can be saved and besides an error in entry can be avoided.
