PROBLEM TO BE SOLVED: To solve problems of a conventional wood combination panel form for foundation concrete supporting the pole of a traffic signal or a road lamp or the like, which is required to be left in an excavated state of a road or the like in about one week from its assembly to demolition, thus causing a danger or the like. SOLUTION: An upper end frame section 3, a lower end frame section 4, and side end frame sections 5 are provided at the peripheral edge sections of a steel sheet corrugated sheet 2 having a plurality of vertical grooves 2a, and reinforcing bars 6 are horizontally provided at the middle portion to form a single unit 1. A plurality of single units 1 are arranged to form a frame body, the side frame sections 5 are adjoined together at a plurality of places, they are connected together with hinges 7 at least at one place, and they are connected together with bolts 8 at least at another place. After the completion of assembly, a hole H is back-filled before concrete is placed, and the frame body is buried in the ground as it is.
