PROBLEM TO BE SOLVED: To provide a form apparatus for a bearing plate, which dispenses with air venting, and harmonizes with an ambient environment, and to provide a method for constructing the form apparatus. SOLUTION: A form main body 10 of the form apparatus is formed by assembling shape steels, and a penetrating portion 31 into which a ground portion 71 of an anchor 7 penetrates is opened in the form main body. Then, thinnings 2 are mounted on surfaces of the form main body 10 except for a surface to be set on a slope surface 8 and the penetrating portion, to thereby construct the form apparatus 1. Further, the ground portion of the anchor is inserted into the penetrating portion, and the form apparatus is set on the slope surface, followed by placing concrete to the interior of the form apparatus.
