PROBLEM TO BE SOLVED: To easily add, change, and delete the kind of readable business form 1. SOLUTION: A business form reader which reads images of different kinds of business forms 1, having data entered by items and outputs the document kinds and data by the items as business form data is equipped with an input item database 35 which stores properties, including the numbers of places of the data by the items 3 of the data entered into the respective forms and a form property database 36 which stores the items embedded in the forms and the entry positions of the data of the corresponding items by the forms 1; the kind of the forms is decided from the read image 28, and a code input image 33 for the respective items embedded in the forms of the decided kinds is generated by using the stored information of the document property database and input item database and displayed out 30, and data by the items which are inputted by using the displayed code input image 33 are regarded as document data.
