SOLUTION: The optical disk having information recording area is provided with grooves or lands as servo information for tracking, a substrate 205 provided with the pits as format processing information for formatting the above information recording area, and a recording film 206 formed on the surface provided with the above servo information and the above format processing information in this substrate 205. ID information is recorded on the above recording film of the information recording area, and the formatting is carried out.
