PROBLEM TO BE SOLVED: To obtain a slip for a detailed statement of pay wherein the detailed statement of pay to be issued each month to a payee from a payer is entered by a dot printer. SOLUTION: A form space 20 wherein the name of the payee, the name of the payer and the detailed statement of pay are to be printed by the dot printer is printed on the surface of the first single-piece slip A of four stacked sheets of single-piece slips A, B, C and D. A form space 22 wherein the name of the payee and the name of the payer are to be printed in a copying manner by the dot printer is printed on the surface of the second single-piece slip B. An advertisement is printed on the rear of the third single-piece slip C. A form space 50 wherein the name of the payee, the name of the payer and the detailed statement of pay are to be printed in a copying manner by the dot printer is printed on the surface of the fourth single-piece slip D. The surface of the slip C is stuck on the rear of the slip B. The right and left side edges 15 of the slips C and D and the upper end edges 16 thereof are stuck together respectively so that they can be cut off. The lower end edges 18 of the slips C and D are stuck together so that they cannot be cut off.
