PROBLEM TO BE SOLVED: To realize the miniaturization of a device while realizing cost reduction by reducing the number of used flexible base boards. SOLUTION: A lens holder 1 holding an objective lens 2 is supported by a total of four wires 7 of both sides in a cantilever state, and electric conduction is performed to a focus coil and a tracking coil from an actuator substrate 9 through the four wires 7. A lens actuator 11 which is an assembly of these parts is held by an optical base 12, and is driven in the radial direction of a disk by screwing a feed screw 14 in a feed screw guide part 13 of one end of the base 12. A flexible base board 17 for turning on electricity to a laser unit 15 in connected to the optical base 12, and the terminal 9a of the actuator substrate 9 is connected directly to the flexible base board 17 through the opening part 12a of the optical base 12.
