PROBLEM TO BE SOLVED: To provide an optical code storage medium readable without raising the working precision, having a variety of combination patterns and capable of being more enhanced in security. SOLUTION: This storage medium comprises a card base material 11 and a hologram 20 that is an optical code recording part provided on the card base material 11 and having a plurality of mechanically readable code parts 20C having a light diffracting structure. The card base material 10 comprises a transparent or translucent light permeable part 10A in the part where the code parts 20C are provided, and the hologram 20 comprises a data area ED where a prescribed number of the code parts 20C are broken, and an unique code is imparted thereto. |
