PROBLEM TO BE SOLVED: To form a concrete constituent object such as a foundation having recessed and projecting patterns on its surface by a simple configuration. SOLUTION: A plurality of forms 1 are mutually connected to constitute a form device 2. Longitudinal slits 3 are provided at a fixed interval in the form device 2. A sheet 5 having recessed and projecting parts 4 for providing patterns is provided along the surface on an inner face of the form device 2, and both end parts of the sheet 5 are inserted into both slits 3, respectively, and are led to an outer face side of the form device 2. A holding means 6 for holding the sheet 5 led to the outer face side of the form device 2 is provided.
