<p>PROBLEM TO BE SOLVED: To make it possible to correct an electronic clinical chart, to easily make an entry into the electronic clinical card, to prevent the electronic clinical chart from being altered, and to speedily adapt to the revision of the law and regulations. SOLUTION: A plurality of treatment plans are held for one mouth to make medical treatment efficient. When a clinical chart is corrected by deleting a line of the clinical chart, the data are not deleted but a double line is drawn over the line. A hand-written illustration can be inserted. In this case, text data at the pasting place are so shifted not to overlap with the illustration and divided by the illustration. If the illustration cannot be put in the remaining part of the page of the clinical chart, a new page is inserted and the illustration is drawn.
