PROBLEM TO BE SOLVED: To provide a form generating device for efficiently generating different kinds of manifests concerning one industrial waste. SOLUTION: A form style storage part 101 stores the output styles of a plurality of manifests. The output styles are generated for every item and id is added to each item. An item data storage part 105 stores item data to be described in manifest items for every industrial waste by making a pair with id. An operator selects one form style from a form style selecting part 106. A control part 107 reads item data corresponding to each item of the selected form style by id from the item data storage part 105, allocates them and reports them to a printing part 108 together with a form style. The printing part 108 prints the reported manifests.
