PROBLEM TO BE SOLVED: To reduce trouble such that an expensive delivery product is stolen during delivery work, in a delivery slip for COD sale in which a delivery charge is entered. SOLUTION: The delivery slip 1 is formed by connecting a main pasting slip 5 and a sub-pasting slip 6 to both sides of a receipt/delivery slip part 2, wherein a receipt 7 and a delivery slip 8 are formed so as to be demarcated by a cutting perforation 9, through folding and cutting perforations 3 and 4 and providing predetermined entry columns on the surfaces of the respective slips 5, 6, 7 and 8. When the delivery slip is folded in three zigzag so that the sub-pasting slip 6 becomes the uppermost position, the receipt/delivery slip part 2 is superposed on the main pasting slip 5 and the sub-pasting slip 6 to make only the entry column of the sub-pasting slip 6 visible and a pressure- sensitive adhesive layer covered with release paper is provided on the back surface of the main pasting slip 5, and the respective sides crossing the folding and cutting perforation 3 of the sub-pasting slip 6 are extended from the receipt/ delivery slip part 2 and the main pasting slip 5, and pressure-sensitive adhesive layers covered with release papers are provided on the back surfaces of these extended parts.
