PROBLEM TO BE SOLVED: To provide an optical information recording and reproducing device having high data reliability and large capacity. SOLUTION: The optical information recording and reproducing device has a helical or a concentric circle-shaped track guide groove preformed by continuous recessed and projecting parts in only a data part of an optical information recording medium and when the recessed part and the projecting part are defined as a groove part and a land part, respectively, an information track is composed of both groove and land parts and header information including address number and sector number is recorded beforehand as intermittent recessed and projecting parts (prepit) in only either one of the groove or the land part.
