SOLUTION: In a permanent type form K equipped with form panels 10 arranged face to face at a predetermined intervals and connecting members 20 connecting the form panels 10 together, the form panel 10 is formed with a surface member 11, a plurality of wood laths 12 arranged in parallel at predetermined intervals on the rear surface of the surface member 11 and insulation members 13 attached to the rear surface of the surface member 11, and fastening pieces 15 are attached to the wood laths, connecting members 20 are provided at the inner side of the form panel 10 and have hook portions 21 to be linked to linking pieces 15, these hook portions 21 are linked and the wood laths 12 are made connectable each other, fluid concrete 3 is flowed between the form panels 10, and the form panel 10 and concrete 3 are built to one united wall body H.
