A system and method for obtaining controlled information delivery of many kinds from a test taker. In one aspect, the system and method provide a medical test device delivery, retrieval and data distribution system (54, 56, 68, 60, 62, 72) in a secure and efficient format. In a further aspect, the invention comprises a method for distributing a test device and providing test results to authorized users.
