PROBLEM TO BE SOLVED: To provide a form booklet coping with both of easiness in mechanical treatment and handling convenience in the observation of printing contents. SOLUTION: A second sheet of FAX/OCR paper S2 is employed for mechanical treatment such as FAX/OCR or the like. In a first sheet of memorandum S1 of customers, into which the letters are written directly by hand, a writing column, an illustration expression or the like can be printed thick and clearly whereby the same can be seen easily while the abstraction of member's regulation or the like can be printed on the rear side of the same with a sufficient thickness. Accordingly, the handling convenience, excellent in writing, preservation and the like, can be realized in the customer's memorandum S1 of the first sheet. In the second sheet of FAX/OCR paper S2, the printing on the front side is secondary whereby printing can be effected with a sufficient thin color compared with that of letters transferred to the FAX/OCR paper S2 whereby the accuracy of optical automatic letter recognition can be secured.
