PROBLEM TO BE SOLVED: To easily remove foreign matter on an information signal layer and to restrain/prevent minute projecting faults from being generated on a light transmitting layer. SOLUTION: The information signal part is formed on a replica substrate by a vacuum deposition method or a sputtering method. An ultraviolet-curing resin 13 is applied on the information signal part to bury the foreign matter onto the inside of the resin 13. A film 3a is stuck to the resin 13 to cover the whole area of the resin 13 while the film 3a is made to travel by a film traveling unit 4. The resin 13 is cured by being irradiated with ultraviolet rays through the film 3a. The cured resin 13 is peeled off from the information signal part. An adhesive ultraviolet-curing resin is applied to the information signal part. A light transmitting sheet is adhered to the information signal part while interposing the adhesive ultraviolet-curing resin, which is then cured to form the light transmitting layer consisting of the cured resin and the light transmitting sheet. The recording/reproducing of an information signal is carried out by irradiating the information signal part with a laser beam through the light transmitting layer from the information signal part-formed side.
