PROBLEM TO BE SOLVED: To provide an optical disk holding sheet wherein a side of an information readout surface of an optical disk can be sufficiently protected, the optical disk can be simply taken out when using or disposing of the disk, it is simple to package the optical disk, and the sheet can be simply attached to a book or the like. SOLUTION: The optical disk holding sheet includes a mount 11 equipped with a cut 11a, a surface-protective film 12 having adhesion of being capable of sticking when pressed against an adherend wherein the film is stuck to cover at least the cut on one of surfaces of the mount, and the optical disk 13 which is placed in the cut of the mount and has the side of the information readout surface stuck to the surface=protective film.
