To bond a woven or tissue material to an underlay by stitching, an opening is made in it when in place for a thread to be passed through and fixed under tension. The fixed thread is additionally tautened when the material is advanced. The process is repeated until the material and the underlay are securely bonded to each other. On bonding tissue to an underlay, the underlay material is cartilage, bone, or tissue which can be hard and/or soft. The space between the tissue overlay and the underlay can be filled e.g. with a cell suspension, a sodium chloride solution, active and/or filling materials. The thread is held in place and fixed by a ball, pin, bolt and the like to give a positive and/or force fit lock. The thread can also be fixed in place by an anchor and/or dowel. The thread and/or lock are of a material which can be resorbed. The thread can also be fixed in place by the filling material and/or an adhesive. The thread locks can be attached to the thread as beads. An Independent claim is included for an apparatus to bond tissue (14) to an underlay (13) with a shaft in the instrument to take a tool (7) such as a needle, drill, miller, die and the like to form a hole in the tissue and the underlay. A feed channel takes the thread (5) from a thread supply. A feed channel delivers the thread fixing units (6) to secure the thread in the opening from a supply. The feed openings of the channels form a common outlet opening channel. A drive is connected to the tool to apply a defined pressure to it. An operating unit triggers the fixing unit, for the thread advance and its tensioning. Preferred features: The tool drive is a mechanical, hydraulic, pneumatic, electrical drive, or a combination of drive systems. The fixing unit is a ball, bolt, or pin. It can also be of filling material and/or elastic bodies and/or an adhesive, an anchor and/or a dowel.
