PROBLEM TO BE SOLVED: To provide an opaque concealing form sheet to a transmission light used for a scratch type public lottery or the like without impairing printability and a drape as paper by incorporating a feature capable of recycling or incinerating. SOLUTION: The concealing form sheet is manufactured by blending a carbon black with an adhesive and adhering at least two or more paper as at least one adhesive layer. In this case, the adhesive layer as a concealing layer contains a solid content coating amount of the carbon black of 5 g/m2 or more. The solid content weight ratio of the carbon black to the adhesive layer is in a range of 30 to 60%.
