PROBLEM TO BE SOLVED: To provide a print head 4 and an image forming apparatus capable of preventing a tailing phenomenon or clogging of toner 1 in a toner passing hole 6. SOLUTION: This image forming apparatus comprises a toner carrier body 2 for conveying the toner 1 by carrying it and the print head 4 that has the plurality of toner passing holes 6 for passing the toner 1 therethrough and controls the passing of the toner 1 by applying an image signal to a control electrode provided to the periphery of each of the toner passing holes 6. A control electrode 7 and a deflection electrode 10a, 10b are provided to both faces of an insulation substrate 12 forming the print head 4, respectively. The toner passing hole 6 is formed in a tapered shape such that the diameter at the side of the deflecting electrode 10a, 10b is greater than that at the side of the control electrode 7. As a result, since the speed of the toner 1 is not reduced by preventing the toner 1 from colliding with the inner wall face of the toner passing hole 6, the length of the toner flow is not enlarged so that the tailing phenomenon can be suppressed.
