SOLUTION: This medium has a questionnaire slip preparing function A used at the time when various questionnaire surveys are performed, a function C for supporting a questionnaire survey on the Internet, a function D for supporting a questionnaire survey using a recording medium such as a CD-ROM, a function for performing a questionnaire survey with any or a combination of a plurality of sets of answer inputting functions E, a function F for browsing answer data collected or inputted by performing the questionnaire survey and a function G for accumulating and analyzing the answer data collected or inputted by performing the questionnaire survey, and a program for realizing these functions by the computer is recorded.
