A computerized disease management method utilizes periodic interactive dialogs with the patient to obtain health state measurements from the patient, to evaluate and assess the progress of the patient's disease, to review and adjust therapy to optimal levels, and to give the patient medical advice for administering treatment and handling symptom flare-ups and acute episodes of the disease. As a given patient is managed over time, a profile in the form of the frequency and reasons for the patient's contacts, the patient's subjective understanding of the disease, the patient's objective response to various medical treatments, and the patient's preferences in treatment, is built. To optimize the therapy for the patient's disease at a particular time, patient health data is obtained from the patient responses in the dialog and from objective health measurements and stored against the patient's health data in a medical record. A particular health parameter from the patient medical record is selected and changes in the patient health state derived from prior disease management sessions, as indicated by the selected parameter, are calculated. The health state changes are compared against disease specific changes for the patient disease, and in accordance with this comparison and the obtained health data, information regarding the adjustment of the therapy is generated.
