PROBLEM TO BE SOLVED: To provide a delivery slip friendly to an environment and suitable for a high speed laser printer by incorporating a function similar to that of a conventional adhesive bonding type multi-part delivery slip without using an adhesive sheet (tacky sheet) by the type of releasing one block and carrying it back after its delivery by dividing one form sheet into a plurality of blocks via perforations and adhering the two of the blocks to a luggage. SOLUTION: In a remoistening type delivery slip, the form sheet is coated with a water soluble remoistening adhesive in a predetermined pattern, dried, and divided into an adhesive block and a non-adhesive block. Then, both the blocks are adhered to the luggage by moistening, and the latter can be released as needed.
