The instrument has a holder (1), a laser light conducting part (3), a freely rotatable output coupling element (15) and a laser light deflector. A treatment liquid feeder (32) has one or more output openings (33) that open in an edge region enclosing the coupling element or in a region bounding on the casing of the coupling element or in a wall hole accommodating it or in the inner surface of a hole (17) accommodating the coupling element.
