PROBLEM TO BE SOLVED: To provide a delivery slip capable of being released from a sheet member easily and surely pseudo-adhered even by picking up any part of a release starting end of a slip body. SOLUTION: The delivery slip 1 is formed by pseudo-adhering a resin sheet 17 to a rear surface side of the slip body 5 having an adhering slip for displaying delivery information necessary to a delivering work on the front surface side and a delivery slip and receipt slip 3, coating an opposite surface side of the sheet 17 to the body 5 with an adhesive, and coating the adhesive coating surface with a release sheet. In this case, an edge 17a of the sheet 17 at the release starting side is disposed at an inner side from the release starting end of the corresponding body 5, and formed in a circular arc-like state bulged toward its outside.
