PROBLEM TO BE SOLVED: To provide a form for concrete capable of seeing through a state of the form from the outside when the ready-mixed concrete is poured into the form in building construction or the like. SOLUTION: The form for concrete is constituted of a rectangular plate body 1 having a plurality of bolt through-holes 2 having an appropriate thickness at specific intervals, a thick edge frame 3 provided at the circumferential edge of the plate body 1 and a plurality of reinforcing members 4 provided on the surface of the plate body 1 surrounded by the edge frame 3, and the plate body 1, edge frame 3 and reinforcing members 4 are monolithically formed of a transparent or semitransparent synthetic resin.
