PROBLEM TO BE SOLVED: To provide a form for a slope surface, which can easily and accurately implement greening of the slope surface, and to provide a form constructing method using the same. SOLUTION: Reinforcements are arranged like a grid pattern on the slope surface, and then a plurality of plastic form members 2 are assembled on the periphery of each grid frame, to thereby prepare the forms 1. The forms are arranged and temporarily fixed onto the slope surface, and mortar or concrete is sprayed to gaps between the respective forms, and then cured. After the curing, the forms 1 are disassembled and removed, and planting is performed in areas corresponding to the forms, to thereby implement greening of the slope surface. With respect to the form, the adjacent form members may be connected together by means of a detachable connector 3, and particularly they may be mutually rotatively connected together. Further, a serrated portion 6 may be formed in each form member at a lower portion thereof, so that the form can perform venting even in a state where the form is fixed to the slope surface, and can be engaged with or hooked to the same.
