PURPOSE: An apparatus and a method of a questionnaire using a smart card are provided to improve reliability in a responsor, and use information efficiently by analyzing question targets in advance and then classifying them. CONSTITUTION: Smart cards(C1-Cn) store user authentication information. Card readers access the user authentication information from the smart cards(C1-Cn), and are connected to keyboards(K1-Kn). A control part transmits the user authentication information read from the card readers to response computers(PC1-PCn) via a keyboard connection line. A question server(20) extracts users from a customer database(32) on a basis of the participation qualification data stored in an order database(30). The question server(20) counts the number of question target persons. The customer database(32) stores the user authentication information and additional information.
