PROBLEM TO BE SOLVED: To provide a delivery slip so contrived that a part of a sheet constituted of two layers, the upper and the lower, is made peelable, this part is cut off with the two layers held as they are after data are printed, and is stuck on a package and the upper layer of the slip being stuck can be peeled off after delivery. SOLUTION: The sheet having the two layers is divided into a plurality of sections by perforations 3-1 to 3-3. The upper and lower layers of the section to be stuck on a package are stuck together by an adhesive making it possible to peel them off layer, and further a dry part F of a rehumidifying adhesive is provided on the rear of the lower layer. This layer can be stuck on the package by humidifying the dry part.
