PROBLEM TO BE SOLVED: To provide a form device wherein workability can be raised, and dimensional precision of a product can be secured. SOLUTION: A form device is used when a synthetic segment is manufactured by placing concrete in a steel shell wherein five faces such as two side faces positioned in an axial direction of a tunnel, two end faces positioned in a peripheral direction of the tunnel and an out side face positioned in a radial direction of the tunnel are prepared with a steel plate. The form device is equipped with a base having a pad on which the steel shell (20) are mounted, a plurality of pillars rising from the base, a first edge fixture (50) separably connected to an edge part of two side faces (24) of the steel shell, a second edge fixture separably connected to the edge part of two end faces of the steel shell, and an attachment (70) wherein the first edge fixture is separably connected to respective pillars (44 and 49) without looseness.
