PROBLEM TO BE SOLVED: To provide a composite sheet which can secure the identified explicit effect, while securing the security effect upon sending. SOLUTION: A simplified health examination sheet 1 is sold, for example, in convenience stores or the like, and examinees who are to be submitted to the simplified health examination purchase it. After purchasing, the examinee tears a perforation 6 to stripp off a surface substrate 4, and remove a test paper 14 from a back-surface substrate 3 to take it out from a space 13. Then, a specimen of blood or the like is attached to the test paper 14 thus-taken out, and the resultant paper is applied on the back-surface substrate 3 in the space 13 by means of a pressure-sensitive adhesive 15 that can be peeled-off repeatedly and applied repeatedly. Then, a release paper 12 is peeled off and the underside of the surface substrate 4 is pressed on a highly pressure-sensitive adhesive 11, so that the simplified health examination sheet 1 is sealed again. The simplified health examination sheet 1 thus-sealed is sent to an appointed examination institution.
