PROBLEM TO BE SOLVED: To eliminate the possibility of separation of an affixing slip part, to make a peeling slip part easy to peel off and thereby to realize reliability and convenience together. SOLUTION: This delivery slip has a slip base 11 which has a tag slip part 11a being an affixing slip part and an undertaking slip part 11b, a transit slip part 11c and a delivery slip part 11d being peeling slip parts, a peeling layer 12 which is formed in the shape of a pattern on the lower side of the peeling slip part of the slip base 11, a resin layer 13 which is formed on the lower side of the slip base 11 or the peeling layer 12 and an adhesive layer 14 which is formed on the lower side of the resin layer 13. The peeling layer 12 is printed with ultraviolet-curing resin, being registered with the peeling slip part.
