PROBLEM TO BE SOLVED: To provide distribution cut forms capable of being all released without impairing appearance of a luggage such as a breakage of a packaging sheet, contaminated residue of a released mark or the like of the forms adhered to a material to be adhered such as the luggage or the like and being treated not to inform a destination's information even by delivering or distributing the luggage to another person by a luggage's receiver. SOLUTION: The distribution cut forms comprise a base material formed separably to a first base material and a second base material by a cutting line, an adhesive layer provided at the lower layer of the base material and having transparency for adhering to the material to be adhered, a separate layer provided between the base material and the adhesive layer and having transparency for separably connecting the base material to the adhesive layer, and a release member provided on the lower surface side of the adhesive layer to cover the adhesive layer and separated when the material to be adhered is adhered.
