PROBLEM TO BE SOLVED: To provide an information transmitting and reserving system with map information, with which receiving job of a dental consultation organization can be speedily completed and a user can easily arrive at the institution on the basis of attached map information by receiving a schedule reservation on the basis of disclosed information provided on a network and extracting, filling and carrying an information presentation form (pre-examination questionnaire) with map until the reserved date before visiting the dental consultation organization when the schedule reservation is completed. SOLUTION: Concerning an information transmitting and reserving system 1 with map information, desired schedule reservation are received from user terminals 9a, 9b and 9c corresponding to the schedule conditions reserved in a schedule reservation DB 4, the schedule reservation, which is not overlapped with this condition, is accepted. In the case of executing reserved contents when the schedule is completely reserved, information presentation form, with which the information to be transmitted is previously made prescribed form, is provided by an information disclosing means 3. |
