PROBLEM TO BE SOLVED: To provide forms equipped with an erasable type leuco dye layer wherein information marks which are not completely be erased are not conspicuous. SOLUTION: A camouflaged layer 4 wherein information marks not completely be erased is made difficult to be visually recognized is formed on erasable type leuco dye layer 3 which is formed on forms substrate 2, the information marks not completely be erased are camouflaged by the camouflaged layer 4, and a state wherein information marks are apparently erased can be obtained.
