PURPOSE: An integrated label type index card is provided to ensure the label function and the index-card function of the index card without the fear of being lost, to generate only a small amount of refuses, to be suitable for the protection of environment, to ,smoothly insert a cartridge into a storage case of a cartridge and to simplify the packaging work. CONSTITUTION: An integrated label type index card(24) is received into an opening and closing typed cartridge receiving case which has a hexagonal shape consisting of a main body having a bottom wall and a cover having a upper wall, and on which an inlet is formed. The index card consists of a rare-side part, a front side part and a folded part, and has cutting edges at both ends of a folding line between the front side part and the folding line.
