A method and apparatus for electronically filling in an on-line form with data associated with a user is provided. A form-originating server location identifier, form location, and user identifier are transmitted to a form-filling server. The form-filling server decodes the location identifier and form location, and opens a connection with the form-originating server. The form-filling server requests and receives the on-line form from the form-originating server. The fields of the form are parsed to identify the fields to be filled in. The form-filling server inserts data associated with a user into the identified fields of the form using the user identifier.
