PROBLEM TO BE SOLVED: To obtain a clean information output without generating a toner stain by preventing an adhesive from being transferred to a drum of a printer by dealing with at a label sheet side. SOLUTION: The method for forming a label sheet comprises the steps of perforating cutting perforations at a label base material 8, perforating cutting perforations 5 at a fill-in cut form 11, superposing the material 8 and the form 11 so that positions of the perforations of the both are aligned, and releasably adhering the material 8 to the form 11.
